With 40 years as a Social Worker, Maraline uses her experience to benefit patients and families of Joliet Area Community Hospice (JACH). Maraline works closely with an interdisciplinary team that includes a doctor, nurse practitioner, nurse, C.N.A., chaplain, volunteers and complementary therapists to provide quality care for our hospice and palliative care patients.
JACH social workers contact each patient and family on admission. An assessment is done of their unique psychosocial needs and a plan of care is prepared in collaboration with the hospice team. The social worker evaluates patients and families to provide support, address end of life decisions and assist with rituals and closure to help with the grief process.
Social workers can also meet with children and provide families information on helping their children. They are encouraged to include rather than exclude them, educate and prepare children at their level of understanding, help with their questions, fears and expression of feelings, and give children choices about participation.
Social workers utilize resources in the community and provide referrals to agencies that provide complementary services to those provided by the JACH hospice team. And social workers help make wishes come true!
In one instance an adult patient surrounded by friends and family in our Hospice Home in her last days of life was given a graduation ceremony and degree from Joliet Junior College. Another degree was bestowed on a pediatric patient in her backyard by the Dean of Illinois State University.
